The photograph was taken by Miss Ethel Thompson, a nurse at the Burbank Hospital and sister to Dr. Elmer Thompson (Thompson Memorial Hospital, and the only doctor in Burbank at this time.) Donor: Hollis Forbes. Black and white photograph, 5 x 7 in.
